VO2 max is the closest thing endurance training has to a single fitness score, which is exactly why it gets over-read. Here is what it measures, how much to trust an estimate made without a lab, and where it stops being the whole story.
What the number measures
VO2 max is the ceiling on how much oxygen your body can take in, move, and actually use per minute, divided by your bodyweight so people of different sizes can be compared. It grades the whole oxygen chain at once: lungs, heart, blood, and the mitochondria inside the working muscle. In a lab it is measured directly from the air you breathe out during a test that ramps up until you have nothing left, which is what makes it the best single laboratory number for aerobic fitness.
How a field test approximates it
The Cooper test drops the mask and infers the ceiling from how far you can run in twelve minutes, on the reasoning that the ground you cover flat out is tightly bound to the oxygen you can use. Kenneth Cooper published it in 1968 after work with US Air Force personnel, and run distance tracked measured VO2 max closely. Against a real lab test, expect error in the region of 10 to 15 percent, and worse if you pace the twelve minutes badly. The Rockport walk asks less of you and is looser again.
A worked example
Say you are a 30 year old man and you cover 1.5 miles in the twelve minutes. The tool converts that to 2,414 metres, subtracts 504.9, divides by 44.73, and returns about 42.7 ml/kg/min, which lands in the Good band for that age and sex. Cover another 100 metres and the estimate moves roughly two points, so read your result as a range rather than a decimal you have to defend.
It responds to training
This is the encouraging part. Max heart rate is largely fixed and drifts down with age no matter how you train, so there is nothing to chase there. VO2 max moves. Starting from untrained, a few months of consistent aerobic work commonly adds around 10 to 20 percent, and the lower the starting point, the larger the jump tends to be. How strongly any one person responds varies a lot, so measure yourself against your own last test rather than against anyone else.
What it does not tell you
Two runners with an identical VO2 max can race very differently. Running economy, how much oxygen you spend to hold a given pace, and your lactate threshold, the hardest effort you can sustain for a long time, decide most of what happens on the day. VO2 max sets the ceiling; those two decide how much of it you get to use. Retest every couple of months and read it next to your training paces, which will usually show progress sooner than the estimate does. Both tests ask for a hard effort, so if you have a heart or lung condition, or you have been away from exercise for a long stretch, talk to your doctor before running one.

Retest every 8-12 weeks; high-intensity intervals 2-3x per week are the fastest way to raise it. Even a 2-3 mL/kg/min improvement is a meaningful fitness gain.
